That is what a tribunal asks. It has two limbs, and making support available does not answer either of them. Knowing and then doing nothing is the worst place to be, which is why the two belong together.

Ignorance is not a defence. An annual survey and a helpline nobody rings are not a system for finding out, and a manager noticing is not a system either.
